Washington State requires a long-term care assessment for every person moving into an Adult Family Home. Ordinarily, this assessment is completed by the move in date. If emergent placement is required, it must be completed within 5 working days of admission.

The assessment includes a thorough review of:

• Social, physical and emotional strengths and needs
  
• Health history
  
• Medications
  
• Activity
  
• Food preferences and special diet
  
• History of anxiety and depression
  
• Behaviors or symptoms that may cause concern or require special care

The long-term care assessment is a necessary tool used by adult family home providers to develop an initial care plan promoting the resident’s optimum level of social, emotional and physical well-being. The assessment also helps the provider in determining the monthly cost for room, board and care.

Sound Transitions will facilitate the completion of the initial long-term care assessment with another qualified, experienced registered nurse or social worker. Fee for this service is available upon request.
