Alzheimer’s Association — Provides services and information to people with memory problems, families caring for them, and health care providers.


American Parkinson Disease Association-Washington Chapter — Helps patients, family members, caregivers cope with Parkinson’s disease. Role is to inform, assist, educate and raise funds to help ease the burden and find the cure.


National Multiple Sclerosis Society (Greater Northwest Chapter) — Educates, inspires and empowers those affected by multiple sclerosis.

Senior Information and Assistance — Senior Services was established in 1967, and is the largest non-profit agency serving seniors in the Seattle/King County area.

Compassion & Choices of Washington (C&C) — sets a new standard in Washington State for health care directives. It is the product of almost a year of committed research and development. It contains several major improvements over the documents that were previously unavailable.

Medicare Prescription Drug Coverage (Medicare D) — Provides information and on-line tools to help navigate the Medicare D plan.


POLST (Physician Orders for Life Sustaining Treatment) Form information — designed to summarize the wishes of an individual regarding life-sustaining treatment. The form is intended for any individual with an advanced life-limiting illness.

Washington State Living Will Registry — a secure, online database that preserves Washington State residents’ end-of-life preferences. The registry is available to authorized health care providers only and contains vital information when a person is unable to communicate or make decisions about end-of-life treatment.
