Placement Service

Sound Transitions’ philosophy is based on establishing trustworthy relationships. I work collaboratively with families, caregivers and Adult Family Home owners to provide choices for families which best match the needs for your loved one needing care.

Sound Transitions has a growing network of Adult Family
homes in King and Snohomish Counties. Each home is visited and assessed by Sound Transitions. Information collected includes: geographic location, acuity of care provided, staffing ratios around the clock, cleanliness, specialty care, prior patient care experience, time in business, price range, activities, amenities and other services provided.

As an RN and owner of Sound Transitions, my primary objective is to assist families in finding placements that will last. I recognize the hardships caused by moving at this time of life and understand the necessity of finding situations that meet as many of my clients’ needs as possible.

HOW SOUND TRANSITIONS WORKS

Once Sound Transitions receives your call requesting assistance, I talk with you about your loved one’s situation and care needs. At that time, I help you decide whether an Adult Family Home is an appropriate placement. If so, I will make an appointment to meet with you, other family members, caregivers, and your loved one.


During our meeting the care needs, medical status and other pertinent information will be reviewed. This is an essential step in identifying the appropriate placement. The goal is to get a comprehensive picture about each client.

This includes:

  1. Obtaining medical Records
    This information enables Sound Transitions to more fully understand the medical issues and health care needs. It is also required information needed to complete the Washington State required long term care assessment for admission to an Adult Family Home. The client or durable power of attorney (DPOA) obtains the applicable medical records for our use or signs a release of information letter if the client is currently in the hospital or a rehabilitation center. 

  2. Learning about personal and cultural preferences

  3. Interviewing caregivers, therapists and the social worker (if your loved one is in a hospital, assisted living facility or rehabilitation center).

  4. Reviewing geographic preferences

  5. Understanding financial restrictions

After the initial meeting, a date is typically made to visit and interview an average of 3 homes selected from the Sound Transitions network. Family members are always accompanied to the homes. I help you ask the right questions of the Adult Family Home owners as you evaluate which setting would best satisfy your loved one’s needs.

Once an Adult Family Home is chosen and your loved one moved, Sound Transitions follows-up with both the family and adult family home owner. I assist with problem solving and mediation as necessary for as long as your loved one is in an adult family home chosen through Sound Transitions.
